Our new care model allows us to be even more accessible using our improved patient portal and phone application. Our dedicated team will assist you with scheduling external appointments, obtaining necessary prior authorizations for procedures and medications, and readily address any questions you may have. We are excited to continue our time at 7900 N. Milwaukee Ave as we move into a newly customized space on the second floor. As you all know, Dr. Herman and Dr. Weiner are the kind of doctors who spend time with you, get to know you and try not to rush your appointment. In the current environment, it is no longer possible for us to continue to provide this kind of care. By eliminating the insurance billing and reimbursement component, it allows us to focus more on you and your health care needs.

In order to be part of our practice, you will pay a yearly fee that will cover everything we do within our office. You will have longer appointment times and easy availability including same-day, in-person, and tele-health visits. We will have a phone and text system where you will be able to reach us when needed. If you need to see us every few weeks or every few months, the fee is the same. Our goal is to limit the number of patients we accept into the practice so we can give quality care to all.

All office visits and contact with the doctors are covered by your fee, including the routine labs for your yearly physical, along with any labs we do in the office (such as COVID tests, urinalysis). Any other lab work will be sent to a reference lab who will bill your insurance directly. Additionally, we will be working with a company who will provide us with vaccines we administer in office, but please note the vaccine company will directly bill your insurance. We plan to start offering additional services in 2026.

Yes, you still need insurance to cover all your other medical care. We will be out of network for all insurances including Medicare, HMOs and PPOs. If you are a member of an HMO we will not be able to do referrals. You can not submit our charges to your insurance for reimbursement.

A concierge physician has the time to spend helping the patient integrate and coordinate the management of their medical issues. Given the current traditional medical structures, it is hard for primary care physicians to focus on you and your issues as we have to satisfy insurance company documentation requirements within a limited appointment time. Your concierge physician has time to address more issues that might otherwise require extra visits to your PCP or other specialists.
Given our greater availability, you will hopefully avoid urgent care and even emergency room visits.

There are some issues that can’t be managed in the office. We do not provide in-office imaging or lab work. If you need an emergency room, please go to the closest one and be sure they send us reports. If you aren’t sure if you need an ER, we are available 24/7 to give you advice.
While we remain on staff at Lutheran General Hospital, we are not seeing patients directly in the hospital. However, we are able to follow your hospital course via the hospital’s electronic record system and be a resource for you and your family while hospitalized.

We will be happy to help you to the best of our abilities. We can have a phone or video visit and may be able to prescribe if necessary. If your problem requires care beyond that, we will advise you what your best options are. If you end up hospitalized, we will be happy to discuss your case with your treating physicians. If you are out of the country, we are unable to prescribe.

Dr. Herman and Dr. Weiner have been working together for over 30 years and have been covering for each other all that time. If one of us is not available, the other doctor will be more than happy to help you. We have remote access to your records so will know your history. As our practice grows, we hope to bring on more physicians.

For current patients and their families, if you join before the end of 2025, the first year fee will be $3,000. For 2026 and new patients it will be $3,600. Young adults 26 and under who have a parent who is a member will pay $500, conditions apply.
